1)    Promote an understanding of health care consent and advance care planning
2)    Define substitute decision makers
3)    Provide an overview of the Ontario hierarchy and the rights of substitute decision makers
4)    Provide an example of appointing a substitute decision maker utilizing a Power of Attorney for Personal Care form
5)    Discuss the importance and benefits of advance care planning
